(AP) ? U.S. Secretary of State Hillary Rodham Clinton says Iran and six world powers have agreed to meet in Istanbul on April 13 for the latest round of talks about Iran's nuclear program.

Clinton made the announcement at a news conference Saturday following a security conference in Saudi Arabia. That conference focused on the nuclear program, which the U.S. and other believe is intended to develop nuclear weapons, and on military threats from Iran in the Persian Gulf region.

Iran and the six nations ? the United States, Britain, France, Germany, Russia and China ? met in Istanbul 14 months ago. But the talks ended after two days with the sides agree on what to talk about.

Iran contends the program is solely for peaceful energy and research purposes.

Law keeps Maine's health insurance rates in check ? Opinion ...

An article in the March 22 edition of the Bangor Daily News with the headline ?Senate Republicans reject state approval for all health insurance rate increases? creates a false impression about Maine law that needs to be clarified. As commissioner of the Department of Professional and Financial Regulation, which includes Maine?s Bureau of Insurance, I appreciate this opportunity to provide additional information.

The article may lead readers to conclude that Maine?s small businesses are not protected against unreasonable health insurance premium increases and that the superintendent of insurance does not have effective rate review authority.

In fact, Maine insurance law has always provided protections for small businesses against unreasonable rates and continues to do so, and the superintendent?s statutory authority to review small group rates is stronger now than it was before the Legislature enacted Public Law 90 last year.

Prior to the enactment of PL 90, Maine law provided that all small group health insurance rates for credible books of business could be implemented without review by the superintendent of insurance provided the insurer guaranteed a 78 percent loss ratio and promised to provide rebates to policyholders if it fell short. This process is often called ?file and use.?

An insurer was only required to get the superintendent?s approval for its small group rates if the insurer would not guarantee at least a 78 percent loss ratio. Further, even when rates are submitted on a ?file and use? basis, they are reviewed by the Bureau of Insurance before being implemented, and they are disapproved if they violate adjusted community rating requirements or other applicable laws.

In 2010, the federal health insurance law ? the Affordable Care Act ? made this guaranteed loss ratio framework mandatory across the entire health insurance market for all insurers with enough covered lives to be statistically credible. For small group insurance, the minimum loss ratio is now 80 percent, although some changes have been made to the formula.

The ACA also now requires prior review of all ?potentially unreasonable? rate increases in the individual and small group markets. Under current federal regulations, ?potentially unreasonable? means an increase of more than 10 percent in one year.

Here in Maine, PL 90, as clarified by PL 364, reconciled the state and federal standards. Maine adopted the federal loss ratio formula and the 80 percent threshold. The 80 percent minimum loss ratio constitutes a significant protection for Maine businesses in the small group health insurance market. In addition, under forthcoming agency rule amendments, the Bureau of Insurance will now conduct prior review of all ?potentially unreasonable? rate requests.

Another important fact not mentioned in the March 22 article is that PL 90 eliminated the ability of insurers with a credible volume of business to avoid complying with the 78 percent minimum loss ratio and the rebate requirement by submitting their rates for prior approval. Now, that option is no longer available in the small group market. Insurers must maintain at least an 80 percent loss ratio and, if they fail to comply, they must provide customer rebates.

As we are witnessing, health insurance policy generates much interest and considerable debate. Regardless of one?s views, however, it is important for people to understand the protections in place to safeguard businesses and others covered by small group health insurance plans from unreasonable rate increases and violations of consumer protection laws.

CINCINNATI (AP) ? Fool me once, the saying goes. But 50 times? That's what a convincing art forger did for nearly three decades when he donated his copies of Picassos and other works of art to unsuspecting museums in 20 states.

Mark A. Landis, who has dressed as a Jesuit priest or posed as a wealthy donor driving up in a red Cadillac, apparently never took money for his forgeries and has never been arrested.

Now his "works" have been collected into their own tongue-in-cheek exhibit, called Faux Real and opening on April Fools' Day at the University of Cincinnati.

Educating people about forgery and letting people know about Landis "is the only way to stop him," said Mark Tullos, director of the Paul and Lulu Hilliard University Art Museum in Lafayette, La., which was duped in 2010 with a donation of a painting supposedly by American Charles Courtney Curran.

Landis creates works in oil, watercolor, pastels, chalk, ink and pencil, making most of his copies from museum or auction catalogs that provide dimensions and information on the originals.

He sometimes bestows gifts under different names, such as the Father Arthur Scott alias used at Hilliard. In that case, he told officials that his dead mother had left works including Curran's oil-on-wood painting "Three Women" and that he was donating it in her memory.

Tullos said museum employees became suspicious when Landis kept changing the subject under questioning. After he drove off, the museum quickly concluded it was a forgery.

To convince museums he is a philanthropist, he also concocts elaborate stories about health concerns, said Cincinnati exhibit co-curator Matthew Leininger.

"He has been having heart surgery for almost 30 years," Leininger said with a frustrated laugh. "This is the strangest case the museum realm has known in years."

Landis, 57, acknowledges what he's up to. He told The Associated Press in a phone interview from his home in Laurel, Miss., that he made his first forgery donation to a California museum in 1985.

"They were so nice. I just got used to that, and one thing led to another," he said. "It never occurred to me that anyone would think it was wrong."

The Cincinnati exhibit of about 40 works given to 15 museums grew to around 100 when Landis donated 60 pieces he possesses, along with his priest's outfit.

The Faux Real show will run through May 20 at the Dorothy W. and C. Lawson Reed Jr. Gallery. It depicts famous art forgers, details of how Landis made some donations and ways of detecting fakes. Visitors can view some works under ultraviolet light that causes sections to glow if they contain contemporary ingredients.

Art experts say not accepting payment for his forgeries has helped keep Landis from being charged with a crime. Museum officials say forgeries can hurt their reputation and cost time and money researching suspected fraud.

Landis typically targets smaller museums without resources to thoroughly check donations. While museums don't pay Landis, some treat him to meals, receptions and gifts like catalogs and souvenirs before realizing they were duped, Leininger said.

The exhibit doesn't judge Landis but is using his story to show how forgeries occur and demonstrate that institutions and the public "shouldn't take things at face value," exhibit co-curator Aaron Cowan.

The exhibit won't increase the value of Landis' works ? considered worthless except as educational tools on forgery ? and the curators have heard no objections to spotlighting his works.

Landis won't profit from the show but says it is "nice of them to do this." And though Leininger says he doesn't think Landis can stop, the forger acknowledges that it's harder to fool people now "than the '80s and '90s, when you could just walk in and donate."

$540M lottery has states anticipating tax jackpot

(AP) ? People queuing up for Mega Millions tickets aren't the only ones salivating over the record $540 million jackpot that could be won Friday ? some state governments struggling through lean times know a hometown winner would bring a tax bonanza.

Taxes on a lump-sum payment option to a single winner could mean tens of millions of dollars of badly needed revenue that could go to restore entire social service programs on the chopping block, pay for hundreds of low-income housing units, forestall new taxes or hire more state troopers.

So many tickets have been sold that the jackpot climbed Thursday to the largest in history, according to officials in Rhode Island, one of 42 states where Mega Millions is played. If a lone winner took the lump-sum payout on the jackpot's current amount, it would be an estimated $389.8 million.

"I'd love it if a Rhode Islander wins," said Rep. Helio Melo, the chairman of the House's Finance Committee.

In Rhode Island, when the tax man comes calling for his 5.99 percent, that would mean an estimated $23.3 million, forked over in a single payment.

With it, the state could pay for most of a $25 million bond for affordable housing that voters may be asked to approve this fall. It could also help Rhode Island reach its goal for aid to school districts for the first time. The state, which has a $7.9 billion budget, is $22 million short.

A big lottery windfall wouldn't solve the state's fiscal woes, but it could help chip away at the debt, pay for one-time expenses or delay budget cuts or tax increases ? including on expensive clothing, pet grooming, car washes and taxi fares ? at least for a year, Melo said.

States set their own tax rates on lottery winnings. New York, for instance, charges 8.82 percent, while several, including California, charge none.

Ohio's share of the lump-sum payout would be $23 million, hardly pocket change but still a fraction of the state's $56 billion two-year budget.

"We're not holding our breath waiting for a tax windfall for the state, but we will always root for Ohio and Ohioans and hope lottery luck comes to a Buckeye," said Joe Testa, the state's tax commissioner.

Connecticut would get more than $26 million in state taxes from a winner who takes the lump sum. The money could be used to help fully pay for Gov. Dannel P. Malloy's proposal to channel more money to schools, said state policy Undersecretary Gian-Carl Casa.

It could also help get rid of recently imposed sales taxes on non-prescription drugs, at a cost of $17.2 million, with money left over to hire enough state police troopers to meet the statutory minimum of 1,248, at a cost of $8.9 million.

Rhode Island already has a tax windfall coming its way from two recent Powerball wins. An 81-year-old Newport woman won the Feb. 11 jackpot worth $336.4 million, and the winning ticket for $60 million jackpot on March 7 was sold here.

The Rhode Island Association of School Committees has asked the state to use the $17 million for technology and wireless Internet in schools. That would be in place of a $20 million bond.

Gov. Lincoln Chafee has said the state, which is facing a $117 million budget shortfall next fiscal year, can't rely on those lottery winnings ? and, of course, no state can. But his director of administration recently weighed in, saying of the tax payment: "We're happy to collect it."

(AP) ? Venezuelan President Hugo Chavez is taking issue with the U.S. State Department for posting a statement advising travelers to beware of rampant violent crime in his country.

Chavez said Tuesday in Cuba during an appearance on Venezuelan state television that the South American country's security situation ought to be compared to that of the United States.

"Where would there be greater lack of public security on the streets? In Venezuela or in the United States?," Chavez said. "Let's look at the figures."

The website of the U.S. Embassy in Caracas includes a link to the statement on State Department travel site describing safety threats in Venezuela. The statement notes that violent crime in the country is widespread and says the murder rate has been cited as being among the top five in the world.

The statement refers to increasing numbers of killings and kidnappings in the country, and cites data from the Venezuelan Violence Observatory, a non-governmental group tracks crime. The organization's tally of homicide figures from police totaled more than 19,000 during 2011, and it described the year as the most violent in Venezuela's history.

Chavez's government has not released detailed annual murder statistics in recent years. Justice Minister Tareck El Aissami said in early 2011, however, that the murder rate was 48 homicides per 100,000 residents in 2010. That was one of the highest murder rates in Latin America.

In the United States, the FBI reported in September that for 2010 the rate for murder and non-negligent manslaughter fell to 4.8 per 100,000 people, less than half what it was two decades ago.

Chavez defended his socialist government's anti-crime efforts and said violent crime is a problem "inherited from the capitalist time" before he took office in 1999.

The Venezuelan leader is in Cuba undergoing radiation treatment following an operation last month that removed what he said was a second cancerous tumor. The first was removed last June.

Chavez is running for re-election in October, and he warned that his political opponents could try to foment violence in Venezuela.

"We're the guarantee of peace," he said.

Court weighs making health coverage a fact of life

WASHINGTON (AP) ? Death, taxes and now health insurance? Having a medical plan or else paying a fine is about to become another certainty of American life, unless the Supreme Court says no.

People are split over the wisdom of President Barack Obama's health care overhaul, but they are nearly united against its requirement that everybody have insurance. The mandate is intensely unpopular even though more than 8 in 10 people in the United States already are covered by workplace plans or government programs such as Medicare. When the insurance obligation kicks in, not even two years from now, most people won't need to worry or buy anything new.

Nonetheless, Americans don't like being told how to spend their money, not even if it would help solve the problem of the nation's more than 50 million uninsured.

Can the government really tell us what to buy?

Federal judges have come down on both sides of the question, leaving it to the Supreme Court to sort out. The justices are allotting an unusually long period, six hours over three days, beginning March 26, to hear arguments challenging the law's constitutionality.

Their ruling, expected in June, is shaping up as a historic moment in the century-long quest by reformers to provide affordable health care for all.

Many critics and supporters alike see the insurance requirement as the linchpin of Obama's health care law: Take away the mandate and the wheels fall off.

Politically it was a wobbly construction from the start. It seems half of Washington has flip-flopped over mandating insurance.

One critic dismissed the idea this way: "If things were that easy, I could mandate everybody to buy a house and that would solve the problem of homelessness." That was Obama as a presidential candidate, who was against health insurance mandates before he was for them.

Once elected, Obama decided a mandate could work as part of a plan that helps keep premiums down and assists those who can't afford them.

To hear Republicans rail against this attack on personal freedom, you'd never know the idea came from them.

Its model was a Massachusetts law signed in 2006 by Mitt Romney, now the front-runner of the Republican presidential race, when he was governor. Another GOP hopeful, former House Speaker Newt Gingrich, supported a mandate on individuals as an alternative to President Bill Clinton's health care proposal, which put the burden on employers.

All four GOP presidential candidates now promise to repeal the Affordable Care Act, which they call "Obamacare." Former Pennsylvania Sen. Rick Santorum calls it "the death knell for freedom."

So much for compromise.

Obama and congressional Democrats pushed the mandate through in 2010, without Republican support, in hopes of creating a fair system that ensures everyone, rich or poor, young or old, can get the health care they need. Other economically advanced countries have done it.

Doing nothing is more expensive than most people realize.

Congress found that when the uninsured go to clinics and emergency rooms, the care they can't pay for costs nearly $75 billion a year. Much of that cost is passed along and ends up adding $1,000 a year to the average family's insurance premium.

The overhaul is neither the liberal dream of a single government program supported by taxes and covering everyone nor the conservative vision of stripping away federal rules and putting free enterprise in charge.

The Obama plan relies on private companies plus lots of regulation to make sure they provide basic benefits, keep premiums reasonable, and cover the sick as well as the healthy. That's where the mandate comes in. If insurers must cover everyone, even those with existing medical conditions, healthy people have little incentive to sign up before they get sick.

Insurance companies argue that if only the sick sign up, insurers will go broke. So the law says everybody must have insurance for themselves and their children, or pay a penalty.

Also, because everyone needs health care sometime, if everyone purchases insurance, the price per person can be lower, with the cost of care spread out over many people.

After all, the government requires workers to pay Social Security and Medicare taxes, whether they want the benefits or not.

One argument for the insurance mandate is that the fines are just federal taxes by another name. Another is that it falls under the government's constitutional power to regulate commerce that crosses state borders.

State governments, of course, tell people to buy lots of things, including auto insurance or motorcycle helmets.

"You can always move to another state," said Tom O'Connor, a consultant in Fairfax, Va., who thinks the health care law overreaches. "It's a little more difficult to move to another country."

Many agree.

In an Associated Press-GfK poll, 85 percent said the U.S. government should not have the power to require people to buy health insurance. When the question is worded without the specific reference to federal power, acceptance of the mandate grows a bit, but 6 in 10 are still against it.

Even among those who generally support the health care overhaul, one-third said they are against the insurance mandate.

There's also a significant minority who sees mandates as a cop-out and prefer a government program that covers everyone, Medicare for all.

It's clear that many people do not understand what the law would do or how it would affect them.

Jan Gonzales, an out-of-work bookkeeper in Pablo, Mont., calls fining people for going without insurance "the most ridiculous, asinine thing you ever heard of."

"If I can't put food on the table for my children, how can I pay for health care coverage?" asks Gonzales, who's been without insurance for seven years. "What moron came up with that idea?"

Of course, she might qualify for the law's exemptions for those too poor to pay and for assistance for low-income people, as well as many in the middle class.

There also are some religious exemptions. .

Estimates vary widely of how many uninsured people will get insurance once it's required in January 2014.

About 4 million people would pay a penalty to the Internal Revenue Service for being uninsured in 2016, the Congressional Budget Office estimates.

By 2016, the fine reaches $695 per uninsured adult or 2.5 percent of family income, up to $12,500 per year. The IRS is in charge of the penalties but can't prosecute violators or place liens against them. Its only enforcement option may be withholding money from refunds.

That leaves insurance companies, who stand to gain lots of new customers, worried that people instead will shrug off the weak mandate.

Meanwhile, the state-federal Medicaid program will expand to cover more low-income people, and that's another issue before the Supreme Court, because many states say they cannot afford the extra cost.
